Health Service Policy

  • The University Health Service assists in maintaining and promoting the health of students.
  • The Lehigh Valley campus is equipped with a health center and employs the services of a registered nurse.
  • A local physician is available for consultation and referral.
  • All new students must submit a health history form which is to be completed and signed by your physician or other health care provider.
  • Penn State University policy requires your health care provider to document immunization against measles, mumps, and rubella.
  • If you have not received the health history form, notify the Lehigh Valley campus health service at 610-285-5017.
  • All students who participate in varsity sports must have a physical every two years.
  • The University does not obligate itself for payment of medical services other than those provided by personnel employed or retained by the University and in facilities owned or contracted for by the University.

Health Insurance

  • Accident and sickness insurance is available to all students.
  • This program is sponsored by University Health Service.
  • Enrollment and claim forms are available from the health service office or on the health service pamphlet rack in the concourse.
  • There are enrollment deadlines, so do not delay enrolling.
